Reporting to the Supervisor, Home and Community Care, the Home and Community Care Worker II is a member of the Home and Community Care Team and is in frequent contact with the client and family. The Home and Community Care Worker II provides a key link to observe the client’s condition and any changes in their support network. The Home and Community Care Worker II reports any changes in the client’s condition to the Supervisor Home and Community Care or the Program Supervisor in the community.

The Home and Community Care Worker II ensure that all client, family and program information, records and communication are treated with strict confidentiality; carry out the client care and/or service plans by following the pre-determined task list; assist clients with activities of daily living according to the care plan and task list. This may include bathing, grooming, dressing toileting, providing medication reminders and ambulation, including assistance with walking, climbing stairs and getting in and out of bed; perform specialized tasks or activities of daily living following individualized, client-specific training. These may include special care for palliative, rehabilitation and post-acute clients; prepare nourishing meals and special diets as required; plan meals with the client and assist client with grocery shopping; perform light housekeeping duties; assist in activities of daily living, other than personal care, where the client may have difficulties, such a watering plants, feeding pets, etc.; light laundry and essential mending of clothing for the client; record data appropriately, accurately, completely and in a timely way to ensure clean and accurate client and employee records; observe the client’s condition, needs and environment on an ongoing basis and report changes to the Supervisor Home and Community Care or Program Supervisor in the community; report changes in planned hours of service to the Program Supervisor; follow Health and Safety regulations and protocols; treat everyone in a respectful manner; participate in staff meetings, conferences and in-service training; maintain ongoing written records and call/communities with client; assist with medical interpretation for the Home Care Nurse; and maintain strict client confidentiality.

The knowledge, skills and abilities are typically acquired through a completion of Grade 10 education and two years of home care experience.

This is a Highly Sensitive Position and a satisfactory Criminal Record Check, along with a clear Vulnerable Sector Check is required.

The Official Languages of Nunavut are Inuktut, English and French. Applicants may submit their resume in any of the official languages of Nunavut. Fluency in Inuinnaqtun is required for the position. Knowledge of Inuit communities, culture, land, Inuit Qaujimajatuqangit, Inuktut and experience working in a northern cross-cultural environment are also considered assets.
