Kindred Home Care provides short and long term home support services, and respite care to individuals and families in the areas of personal care, activities of daily living, companionship, medication reminders, end of life care and home management. In a role as Care Coordinator you will play a supporting role to the regional Care Manager. This is a critical support role where you will function as the owner of all scheduling and fulfillment solutions in your region.

Role Responsibilities:

  • Scheduling & Service Delivery Coordination
  • Leadership and support to the Care Support team
  • Ownership over your regions master schedule patterns
  • Ensure fulfillment and accuracy on all anticipated hours
  • Supporting your Care Manager with in home client assessments
  • Establish genuine connections with your team, clients, funders, and caregivers

Qualifications & Requirements:

  • Ability to communicate professionally (oral and written) with French-speaking clients and caregivers
  • Experience with Excel and Scheduling support work is preferred
  • Experience with Salesforce would be considered an asset
  • Excellent critical thinking, organizational and problem solving skills
  • Effective relationship building skills, outstanding communication and interpersonal skills
  • The ability to think quickly, re-prioritize tasks in an ever-changing environment
  • Ability to work from home with minimal supervision
  • Positive, flexible and Reliable

Hours:

The role of Care Coordinator provides support for our 24/7 operations and may share the responsibility of on-call weekend and Holiday support. The majority of your set schedule would be Monday to Friday 7:30am-4:30 pm working remotely from your home. You will also be required to travel throughout the Moncton region to support in-home client assessments as needed.

Job Type: Full-time

Salary: $38,000.00-$42,000.00 per year

Additional pay:
